These plans involve an arrangement between the insurer and a selected network of healthcare providers. The Managed Care Plan provides lower cost in exchange for more limited choice. There are three types of Managed Care Plans:
Health Maintenance Organization (HMO): With this plan, you select a primary care physician who directs your care and refers you to other in-network providers when needed.
Preferred Provider Organization (PPO): This is a combination of an HMO and Fee-For-Service Plan. A PPO offers the flexibility of going out of network, but at a higher cost.
Point-Of-Service: This is a combination of an HMO and PPO Plan. A POS primary care physician can refer you to providers out of network. Again, the increased choice can result in higher cost.
